USCIS-Designated Civil Surgeon
Immigration medical examinations are performed by Brian T. Morris, M.D., J.D., M.B.A., M.P.H., a USCIS-designated Civil Surgeon authorized to complete and certify Form I-693, Report of Immigration Medical Examination and Vaccination Record.
Examinations include a review of your medical history and vaccination records, consultation with the Civil Surgeon, the required physical examination, laboratory testing, and completion of Form I-693.

Schedule your appointment
Immigration medical exams are available by appointment on Mondays, Tuesdays, and Thursdays.
Prepare for your visit
Bring your vaccination records. No fasting is required, so please eat and drink normally before your appointment, and stay well hydrated for your required bloodwork.
Form completion
Applicants are contacted when their completed Form I-693 and supporting documents are ready for pickup, typically within one week of the examination.

Yes. Our exams are completed and sealed by a USCIS-designated civil surgeon, so your I-693 is ready to file.
Do you give the vaccines I need?
Yes. We review your records and give any vaccines USCIS requires during the same visit, so you do not need a second appointment.
